Liposuction, tummy tucks, and fat grafting can produce beautiful results, but not every outcome turns out the way a patient hoped. Lumps, dents, uneven areas, or firm knots under the skin can linger long after the swelling has resolved, and living with them can feel discouraging after you have already invested time, money, and recovery in the first procedure. If this sounds familiar, you are not alone, and many contour irregularities can be meaningfully improved with a thoughtful revision plan.

Revision work draws directly on Dr. David N. Sayah’s research background in wound healing and scar formation, including his published studies on apoptosis in keloid tissue in the Journal of Surgical Research and Plastic and Reconstructive Surgery. A Diplomate of the American Board of Plastic Surgery and a Fellow of the American College of Surgeons, he completed his general surgery residency at NYU Medical Center and his plastic surgery residency at UCLA Medical Center. Over more than 26 years in practice, he has treated a growing number of patients unhappy with body contouring results from surgery performed elsewhere, and he holds active hospital privileges at Cedars-Sinai Medical Center, Saint John’s Hospital, and UCLA Medical Center. Every revision begins with a careful evaluation of your tissue, surgical history, and overall health, because not every patient is a candidate and results vary from person to person.

“I perform liposculpture rather than simple liposuction, approaching each procedure as a sculptor approaches a masterpiece.” — Dr. David Sayah

What Is Body Contouring Revision Surgery?

Body contouring revision is a secondary procedure that corrects irregularities left behind by an earlier liposuction, tummy tuck, fat transfer, or similar body procedure. Rather than starting with untouched anatomy, the surgeon works with tissue that has already been altered. Scar tissue, previously disrupted fat layers, and changes in skin quality all influence what can be safely corrected and how much improvement is realistic.

This is why revision is often more demanding than the original surgery. Dr. Sayah approaches it the same way he approaches liposuction and every body sculpting procedure, as a three-dimensional study of proportion, light, and shadow rather than a single problem spot. Patients whose main concern is a Brazilian butt lift that now feels too large can learn about that specific procedure on the BBL reversal page.

Common Contour Irregularities After Body Contouring

Contour irregularities are visible bumps, dips, or dents in the soft tissue that interrupt the body’s natural, smooth shape. Patients typically come to Dr. Sayah with one or more of these concerns:

Lumps and residual fat:

Areas where fat was removed unevenly, leaving bumps after liposuction or a tummy tuck.

Dents and divots:

Depressions that form when too much fat was taken from one spot, sometimes with the skin tethered to the tissue beneath it.

Asymmetry:

One side of the abdomen, flanks, thighs, or another treated area looks noticeably different from the other.

Uneven fat graft results:

Transferred fat that settled unevenly after fat transfer, creating lumpy or asymmetric areas.

Fat necrosis:

Firm lumps that develop when some transferred fat does not survive, which can often be both seen and felt under the skin.

Poorly healed incisions:

Wide or raised scars from the original procedure, which can sometimes be addressed alongside contour work through scar revision.

Contour irregularities are a cosmetic and sometimes functional concern rather than an emergency. If you recently had surgery and notice fever, spreading redness, drainage, or sudden swelling, contact your operating surgeon right away. Chest pain or shortness of breath after surgery calls for 911 or a visit to the nearest emergency room.

Body Contouring Revision Techniques

Dr. Sayah draws on his reconstructive training to match each technique to the specific problem, and many patients benefit from a combination of approaches in a single procedure.

High-Definition Liposuction for Residual Lumps

For leftover bumps after liposuction or a tummy tuck, Dr. Sayah performs high-definition liposuction with extremely fine cannulas. The smaller instruments allow him to reduce lumps precisely and blend the treated area into the surrounding contour.

Fat Dispersion and Redistribution

When a patient has both bumps and depressions, Dr. Sayah uses a technique he developed called fat dispersion and redistribution. Working with specialized cannulas, he gently loosens fat cells so that tiny fat droplets can shift and settle into the hollow or depressed areas.

Picture thousands of tiny marbles poured onto an uneven floor. The marbles roll into the low spots and fill the dips until the surface levels out, much the way builders “float” a floor with a thin, self-leveling layer of concrete. Redistributed fat follows the same principle, settling where it is needed to create a smoother, more even contour.

Treating Fat Necrosis

Small areas of fat necrosis can often be removed with liposuction or a minor surgical excision. Larger, firm areas usually need to be removed surgically, which can leave a depression where the hardened tissue used to be.

Micro-Droplet Fat Grafting

To rebuild those depressions, Dr. Sayah uses micro-droplet fat grafting. The process works much like a 3D printer, with each tiny droplet of your own fat placed deliberately and artistically to build the contour back up layer by layer until the hollow is restored. Because not all grafted fat survives, some patients benefit from a second, smaller grafting session.

“Revision starts with understanding what the first surgery left behind, the scar tissue, the remaining fat, the quality of the skin. Only then can I tell a patient honestly what can be improved and how much.” — Dr. David Sayah

What to Expect During Body Contouring Revision

Your Consultation

Your first visit focuses on understanding exactly what happened and what is realistic now. Dr. Sayah examines the treated areas while you are standing and lying down, feels for firm tissue or scarring, assesses skin quality, and reviews your prior operative reports when they are available. Imaging may be recommended in some cases to better define firm areas. Patients who live outside Los Angeles can begin with a virtual consultation.

Timing Your Revision

Tissue needs time to settle before a revision can be planned accurately. Swelling after liposuction and abdominoplasty can take months to fully resolve, and an area that looks uneven at six weeks may soften considerably on its own. Revision is generally planned once the tissues have matured, often six months or more after the original procedure, and Dr. Sayah will recommend the right timing based on your exam.

Surgery and Recovery

Revision procedures take place in The Sayah Institute’s fully accredited surgical facility in Beverly Hills, with anesthesia tailored to the extent of the work. Afterward, you will wear compression garments to support the tissue as it heals. Recovery depends on which techniques are used, and many patients who have liposuction-based revisions return to desk work in about a week. Final contours develop gradually over several months as swelling resolves.

Insurance and Cost

Depending on your health plan and diagnosis, some revision and reconstructive procedures may be partially or fully covered by insurance. This applies especially to fat necrosis or deformities that cause pain or functional problems. The team will review your situation and help verify your benefits before treatment, and you can learn more on the body insurance and fees and financing pages.

Frequently Asked Questions About Body Contouring Revision

Most patients need to wait until swelling has fully resolved and the tissue has softened, often six months or longer. Dr. Sayah sets the timing based on your exam rather than a fixed calendar, since correcting too early can make it harder to see the true contour.
